You reach over to grab your phone first thing in the morning, and suddenly the ceiling is spinning. Or you sit up too quickly and feel like you’ve just stepped off a merry-go-round. It passes in a minute, but it’s terrifying while it lasts. This is one of the most classic descriptions of BPPV (Benign Paroxysmal Positional Vertigo) and if it sounds familiar, you’re far from alone. The great news? So What On Earth Is BPPV?
Inside your inner ear there are tiny calcium crystals, think of them like grains of sand that help your brain sense gravity and movement. In BPPV, some of these crystals break loose and drift into the wrong part of your ear. When your head moves in certain directions those loose crystals shift around and send confusing signals to your brain, making it feel like you or the room around you is spinning.
It’s essentially a plumbing problem in your ear. The crystals are in the wrong canal and your brain is getting faulty information. The fix? Moving those crystals back where they belong, which is exactly what our trained physiotherapists can do.
The word “benign” matters here. Despite how frightening it feels, BPPV is not dangerous. It won’t damage your brain, your hearing or your balance permanently. It’s a mechanical problem with a mechanical solution.

BPPV has a very specific pattern that sets it apart from other types of dizziness. The key clue is that it’s triggered by movement – particularly changes in head position.
Sudden spinning sensation triggered by rolling over in bed, sitting up or tilting your head back.
Spinning episodes that last less than a minute, usually 10 to 30 seconds then settle on their own.
Nausea during or after a spinning episode, sometimes vomiting if it’s severe.
A feeling of unsteadiness or “foggy head” between episodes.
Symptoms that come and go, sometimes you have a bad week, then it settles for a while.
No hearing loss, no ringing in the ears, no headache – those point to other causes.
Could It Be Something Else?
Dizziness and vertigo are symptoms, not a diagnosis and they can come from a number of different places. Here’s how BPPV compares to some other common causes:

BPPV
Brief spinning triggered by specific head movements. No hearing changes. Settles within a minute. Highly treatable with physio repositioning techniques.
Vestibular Neuritis
Constant vertigo lasting days, often after a virus. No hearing loss. Usually improves on its own but vestibular rehab with a physio speeds recovery significantly.
Labyrinthitis
Similar to vestibular neuritis but also involves hearing loss or tinnitus. Often viral. Medical assessment recommended alongside vestibular rehab.
Meniere’s Disease
Episodes of severe vertigo with fluctuating hearing loss, ear fullness and ringing. Needs specialist diagnosis and management – physio plays a supportive role.
Cervicogenic Dizziness
Dizziness linked to neck stiffness or poor posture rather than the inner ear. Often accompanies neck pain. Physio addresses the neck directly.
Orthostatic Hypotension
Lightheadedness on standing quickly due to a brief drop in blood pressure. Not true vertigo – more of a “head rush.” Often managed with lifestyle changes.
See a doctor or go to emergency if: your dizziness is accompanied by sudden severe headache, double vision, difficulty speaking or swallowing, weakness in your face, arm or leg or if you’ve had a fall or head injury. These symptoms need urgent medical attention and are not typical of BPPV.
